Internship Confirmation Report: (Turn in at the beginning of the semester). Internship Completion Report: (Turn in at the end of the semester). Time sheets: (Turn one in at the end of every month). Confidential Intern Evaluation (supervisors, fill this out online) Confidential Intern Evaluation Form (MSWord). This document is to be completed by the intern's supervisor and sent directly back to the MTSU Internship Coordinator.
